Following a hugely successful survey last month by The Joint Commission (TJC), Coastal Health & Wellness (CHW) has earned TJC’s Gold Seal of Approval® for Ambulatory Health Care Accreditation and Primary Care Medical Home certification.
The Gold Seal of Approval® is a symbol of quality that reflects CHW’s commitment to providing safe and effective patient care.
CHW underwent an unannounced, comprehensive two-day on-site survey in September for its Texas City and Galveston clinic locations for both accreditation and certification. During the visit, TJC representatives evaluated compliance with care standards related to a variety of areas including coordination of care, infection prevention and control, management of medications, patient education and training.
“This is one of the most successful surveys I’ve participated in. It is one of the best in Coastal Health & Wellness history and that is all due to our team’s outstanding work and dedication to make our clinics the best they can be,” said CHW Chief Operations Officer Ann O’Connell. “We can be, and should be, proud of the care we provide for our community.”
This marks the first Primary Care Medical Home certification for CHW and recognizes efforts to coordinate care with patients and their families.
“It’s common for patients to receive care from more than one provider – from their physician to a nurse, our mental health team, dental team and our lab. Operating as a Primary Care Medical Home allows us to focus on offering patient-centered, comprehensive care,” O’Connell said. “They all work together to make sure the patient’s physical and mental health needs are being addressed from prevention and wellness to acute care and chronic care.”
CHW provides access to high quality primary care to any and all individuals in need and welcomes children, adolescents and adults for all services including medical, dental and counseling care. Same-day appointments are available with clinics in Texas City and Galveston. CHW welcomes Medicaid, Medicare, private insurance and self-pay.
